  • Adding an Accident / Incident

    Click on Report an Accident / Incident on either the panel or the fly-out menu.

    • Enter the details of the Accident / Incident into the form.
    • Additional options are available from the main panel:
      • Administrator
        • Mango enables your organisation to set up multiple administrators. In this field, click on the administrator to whom you wish to send the Accident / Incident.
    • Click on Save.

    Viewing the Accident / Incident Register

    Contains a list of all Accident / Incident raised on the system.

    • Click on Accident / Incident on the left panel.
    • Click on Accident / Incident Register on either the panel or the fly-out menu.
    • All Accident / Incident logged in Mango will be displayed (sensitive data can be hidden).
    • Click on the Accident / Incident you wish to view.
    • The Accident / Incident will open displaying all information and actions to date (in real time).
  • If you are assigned an Accident / Incident to investigate, Mango will notify you via email. My investigations are available through My Zone, or through the Main Menu.

    • Click on My Investigations on either the panel or the fly-out menu.
    • Displayed in the main panel are all of the investigations that have been assigned to you.
    • Click on the Accident / Incident you wish to work on. The Accident / Incident will be displayed in the main panel.
    • Click on Edit on the left panel.
    • Additional options are available from the main panel:
      • Attach Documents
        • You can attach multiple attachments to support your investigation.
      • Actions
        • Record the actions carried out.
      • Investigation is done
        • If you tick this box and the click on Save, the investigation will be sent to the Mango Administrator for review. Please note: if you save but do not tick that the investigation is done, Mango will record your actions to date and the investigation will remain with you so that you are able to add more details at a later date.
  • Re-Opening closed Accident / Incidents:

    • Click on Accident / Incident on the left panel.
    • Click on the Reopen Closed Accident / Incident from either the fly-out menu or the main panel.
    • Click the Accident / Incident you wish to reopen.
    • Type in the reason for reopening (this will be added to the action log of the improvement).
    • Tick the Confirm box.
    • Click on Save.
  • The report writer allows you to create and record report templates within Mango.

    • Click on Accident / Incident on the left panel.
    • Click on the Accident / Incident from either the fly-out menu or the main panel.
    • Create a new report (Add New)
    • Drag any available fields you would like included in the report from the left to the right panel.
    • Double click any field to apply a filter to that field.
    • Click "Run Report" to create a report based on real time system information.
