  1. Incomplete documentation of requirements i.e. not having records/evidence of monitoring results
  2. Not maintaining records of changes/updates to processes or communicating these with the relevant/interested parties
  3. Equipment not being accurately or appropriately calibrated

  5. Controls established to address environmental requirements either not fit for purpose (each life cycle stage should be considered where applicable) or not suitably communicated (incl. contractors/suppliers)
